I hereby apply to become a full member of Daenamoo Taekwondo , fully realizing that Taekwondo is a Martial Art involving contact fighting. If accepted I will indemnify from litigation or prosecution the instructors or any agent of Daenamoo Taekwondo as a result of injury whether accidental or otherwise. I shall further  indemnify and keep indemnified the above mentioned members of Daenamoo Taekwondo  for any situation which may arise from training or championships whilst a member of Daenamoo Taekwondo. I will not use the skills taught to me  by Daenamoo Taekwondo  Instructors unless my personal safety or that of my family are threatened and I will only use as much force as is necessary to stop the attack. I will not use the skills taught to me, outside the academy. I will not behave in a manner so as to disgrace the name of Daenamoo Taekwondo. This membership is subject to being revoked at anytime without  warning and I fully understand that breaking any of the rules or regulations of Daenamoo Taekwondo , Taekwondo Australian Inc., Taekwondo  QLD  Inc , may result in my membership being cancelled.* I also understand that if I leave Daenamoo Taekwondo for any reason I will relinquish my life membership. (*This will only apply to new students who join after 31/01/04)

I have read carefully the above paragraph and fully understand what is required of me as a  Daenamoo Taekwondo Student and promise to uphold the rules and regulations.

Students under the age of 18 require parents permission to attend classes.

Once your enrollment is received, Wayne Wood will contact you to arrange your 2 free introductory classes.
